Coordinates with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

• Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred. * All colleagues must provide licensure or proof of application in process for the secondary state within 6 months from hire date.
• Master's Degree in Social Work required.
• Two years of experience in hospital setting or medical social work setting preferred.
• Experience in case management, addiction services, mental health, or population health management desired.ESSENTIAL FUNCTIONS:
  • Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions. -
  • Provides interventions for patients, families, and caregivers as appropriate. - Interventions may include but are not limited to brief counseling, supportive therapy, crisis intervention, grief counseling, psychiatric placement/referral, alcohol/drug assessment and referral, and referral to alternative community resources.
  • Participates in care coordination activities for those patients identified as needing social work and/or complex case management interventions.
  • Works collaboratively with healthcare team members to efficiently allocate resources.
  • Accurately documents assessments, plans, and interventions in the EMR.
  • Responsible for coordination of care to mental health and/or substance abuse facilities.
  • Responsible for following established patient choice policy.
  • Responsible for coordinating with physicians and other legal entities relating to, but not limited to, guardianships, involuntary psychiatric holds, power of attorney, and advanced directives.
  • Collaborates with physicians and hospital staff through formal and/or informal multidisciplinary case conferences to meet the emotional, mental, physical and spiritual needs of patients and/or their families/caregivers
  • Responsible for weekend and holiday social work coverage.
  • Actively participates in department staff meetings and other teams or committees as required
  • Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, Geriatric as applicable).
