We are seeking to hire a PRN Clinical Resource Manager (CRM) to
provide hospital case management, utilization review, and discharge
planning. This requires a collaborative determination of what level
of care is needed post-discharge, and providing necessary decision
support to patients/families and physicians, while managing patient
and family expectations.
This person will serve as patient advocates--building
collaborative relationships to maximize both the patient and
family's ability to make informed decisions, with the end goal of
ensuring a smooth transition to the next level of care and
services. This includes coordinating the integration of social
services into patient care as appropriate, and coordinating the
hospital activities concerned with case management/utilization
review and discharge planning. To help achieve quality clinical and
cost outcomes, our CRMs incorporate real-time contacts with
physicians, nursing, and ancillary caregivers to establish specific
treatment, cost, and transition targets and to facilitate
Our ideal candidate has strong interpersonal and communication
skills (both verbal & written), and demonstrates leadership,
independent problem solving and decision making abilities. He/she
is able to handle multiple tasks and high volume of work, and has
intermediate computer skills with an emphasis on MS Word &
- Minimum of two years of varied hospital clinical experience
- Experience in case management, home health, and/or insurance
- BSN or bachelor's degree in a related healthcare field
required. Master's degree preferred.
- Colleague must have an RN license, as defined by their primary
work state (Idaho or Oregon), issued prior to hire date.
- Colleague must obtain licensure in the secondary state (Idaho
or Oregon) within six months of hire and cannot perform nursing
care in the secondary state until the secondary licensure is
recorded by our HR team.
