In this role, you will provide personalized rehabilitation services to patients in a supportive and compassionate environment, aiding their recovery from major illnesses and injuries. Our focus is on delivering high-quality care tailored to each patient's unique recovery journey, through a comprehensive approach combining inpatient rehabilitation, home health, and hospice care services. Assess patient condition by conducting physical exams and tests to determine the most effective treatment plans. Develop and implement physical therapy treatment programs based on each patient's specific needs and medical history. Educate patients and families about the recovery process including exercises that can be continued at home. Document patient care and progress, maintaining accurate and complete records in compliance with legal requirements. Collaborate with healthcare professionals to plan and coordinate patient care. Modify treatment plans as necessary, prioritizing patient safety and best outcomes. Strong understanding of physical therapy modalities and techniques. Excellent communication and interpersonal skills to interact effectively with patients and team members. Ability to manage multiple patients with varying health conditions effectively. Knowledge of regulatory compliance and documentation requirements in healthcare. Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapy license. CPR certified.
