Job Description Certification Details - BLS - ARRT(CT) - Medical Imaging License for CT issued by the Oregon Board of Medical Imaging (OBMI) Job Details - Position: CT Technologist - Must be registered ARRT(CT) - Medical Imaging License for CT issued by the Oregon Board of Medical Imaging (OBMI) required Job Requirements - BLS certification required - Registered ARRT(CT) - Medical Imaging License for CT issued by the Oregon Board of Medical Imaging (OBMI) Schedule Information - Contract length: 13 weeks Additional Details - American Heart Association certification required for all direct patient care personnel - Must attach scanned copy of certifications with expiration dates - License or certification verification must be dated within 30 days of start - Fit Test required - 10 Panel Drug Screen required within 30 days of start - Trinity Code of Conduct Form required - Standard i9 completed by agency - Confidentiality and Information System Security Agreement required - Integrity and Audit Services Form signed prior to start - Motor Vehicle Report (MVR) verification within 30 days of start - Specialty exam must be completed within a year - Color Vision test required - COVID-19 Vaccine proof or declination required - MMR vaccination proof required - Hepatitis B vaccination proof required - Influenza Vaccine proof or declination required - 2 Step PPD OR IGRA (Quantiferon/Tspot) required - Varicella vaccination proof required - Physical/Health Statement required - Resume and Skills Checklist within a year required - Conflict Of Interest Disclosure Form required - Criminal Background Check within 30 days required - Education Verification required - EPLS report within 30 days required - GSA/OIG exclusion check within 30 days required - Joint Commission Testing (CORE Exam) within a year required - NSO check within 30 days required - System Access Form must be completed one week before start - Skills Checklist required
