Licensed Clinical Social Worker (LCSW) is an important team member
supporting the Behavioral Health Department, which is essential for
achieving the dual mission of Full Circle Health. The LCSW is a
licensed independent provider who shall be accountable for 1)
maintaining a panel of patient referred from primary care providers
across Full Circle Health clinics, 2) staffing the Behavioral
Health Consultation Blast Line as dictated by the schedule created
and maintained by the Behavioral Health Manager, and 3) provide
consultation as needed about behavioral health patient issues and
needs to primary care providers and nursing staff.The LCSW works
closely with the Integrated Care Team members as well as team
members from the medicine and nursing departments.Benefit
