Job Description:
Job Description Certification Details - BLS - State Physical
Therapist License Job Details - Critical access rural hospital. -
Large portion of inpatient and swing bed rehabilitation. - The rest
is outpatient, which consists primarily of orthopedics and
potentially some neuro depending on experience. Job Requirements -
American Heart Association BLS certification required for all
direct patient care personnel. - License/Certificate Verification
must be dated within 30 days of start. - Specialty exam competency
testing must be completed within a year. Schedule Information - 13
weeks contract length. - 40 hours per week. - Day shift from 07:30
to 18:00. - Occasional Saturday coverage. Additional Details - Fit
Test required. - Trinity 10 Panel Drug Screen within 30 days of
start, preferred within 14 days. - Trinity Code of Conduct Form
required. - Trinity I9 form required. - Confidentiality and
Security Agreement required. - Integrity and Audit Services Form
signed prior to start. - Motor Vehicle Report dated within 30 days
of start. - Color Vision test required. - COVID-19 Vaccine proof or
declination required. - MMR vaccination proof or IgG titer
required. - Hepatitis B vaccination series required. - Influenza
Vaccine proof or declination required. - 2 Step PPD OR IGRA test
required. - Varicella vaccination proof or IgG titer required. -
Physical/Health Statement within last year required. - Trinity
Profile including resume, skills checklist, and references
required. - Conflict Of Interest Disclosure Form required. -
Criminal Background Check within 30 days required. - Education
Verification required. - EPLS report within 30 days required. -
GSA/OIG exclusion report within 30 days required. - Joint
Commission CORE Exam within a year required. - NSO report within 30
days required. - System Access Form to be completed one week before
start. - Trinity Skills Checklist for clinical contractors.
